INTEGRATED MODEL OF CARE
Salud provides integrated medical, dental, pharmacy, and behavioral health primary care services to the communities we serve. Behavioral Health Providers are part of the medical team, and will be available at your medical visit to support you with your needs.

ANNUAL EMOTIONAL CHECK-UPS
Salud offers annual behavioral health screenings and emotional check-ups as part of your medical visits. If you have not seen your Behavioral Health Provider in the last year, follow up with your medical team to schedule an appointment.

IDENTIFYING BEHAVIORAL HEALTH NEEDS
If members of your medical team identify a behavioral health need, they will initiate a consultation with a Behavioral Health Provider (BHP). The BHP will then conduct a brief assessment. The BHP will also provide recommendations for treatment and support to the patient and medical team. Behavioral health needs can include:

  • Adjusting to a new medical diagnosis

  • Management of chronic conditions

  • Stress and/or specific concerns about family or work that are affecting your health and wellbeing


MEDICATION ASSISTED TREATMENT (MAT) AND SUBSTANCE USE SERVICES
Salud offers outpatient treatment for substance use, including Medication Assisted Treatment (MAT), which involves the use of medications to prevent withdrawal symptoms. Salud provides medications for opioid (Buprenorphine/Suboxone) and alcohol (Vivitrol) use. Medical and Behavioral Health Providers work together to support the recovery process of problematic use of substances and mental health needs. We offer both in-clinic and telehealth services for this program.

TELE-BEHAVIORAL HEALTH
Salud provides behavioral health services by phone or video, with the goal of increasing access and offering more flexible care. If you are an established Salud patient, you may receive a call to connect with a Behavioral Health Provider (BHP) for a wellness check-up. Other types of services, such as individual therapy, can also be conducted by phone or video. Please discuss these options with your BHP.

DIVERSE TEAM OF BEHAVIORAL HEALTH PROVIDERS
Salud's Behavioral Health Providers (BHP's) have different educational backgrounds: clinical social work, counseling, clinical psychology, substance use specialists, and family and marriage therapists. Many BHP's are bilingual and are able to offer all services in Spanish. Some of Salud's BHP's are bicultural, occasionally matching the cultural background of the patient and enhancing the cultural awareness of our behavioral health team.

SERVICES OFFERED

  • Behavioral health screenings and outreach

  • Behavioral health consult

  • Brief therapy sessions (15-30 minutes)

  • Extended therapy sessions (50 minutes, 4-8 sessions total)

  • Substance use treatment

  • Referrals and case management related to specialized behavioral health services

  • Health coaching and any support needed adjusting to medical status (e.g., chronic illnesses such as diabetes, chronic pain management, and others)
